Sunrise Family Foot Care Center always Welcomes New Patients!

To schedule an appointment you may call our office at 954 748-9444 or request an appointment on line.

All new patients must complete a Patient Information form and a Medical History form. To save time, we recommend that you complete these forms prior to your visit. Simply choose an option listed below, fill out the forms as completely as possible, and submit them online or bring them with you to your scheduled appointment. Also, don't forget to bring your insurance cards with you. Co-pays and deductibles are also due at the time of your appointment.

Option 1: Patient Information Form Online

This option lets you enter your patient information and medical history directly online. After you enter all of your information, click on the 'Submit Form' button and your information will be sent to our office electronically!

This option will allow us to prepare your file, verify your insurance information in advance and save you significant time on your initial visit.

Option 2: Printable Versions
If you have a printer, you can print out our Patient Information form and our Medical History form. Once printed, bring the completed forms to our office on your initial visit.

Step 3: At your convenience, complete all the information on each form. Please provide both completed forms when you arrive at our office. Completing the Patient Information and Medical History forms in advance will save you time on your initial visit!
